Coaching Disclaimer:

• ☑ I acknowledge that I am responsible for my own choices, actions, and results. Blake Levinson provides guidance, but ultimately, my decisions are my own.
• ☑ I agree not to hold Blake Levinson liable for any outcomes—whether they involve career changes, unexpected epiphanies, or spontaneous dance parties.
• ☑ I understand that all coaching sessions are confidential, except in cases where disclosure is required by law.
• ☑ I agree to provide at least 24 hour-advance notice if I need to cancel or reschedule a session. Otherwise, the session fee may still apply.
• ☑ I acknowledge that all payments are final & non-refundable once a session is finished.
